North Tyneside Transition Information

Valuing People Now recognises that as young people grow up getting the support they and their family needs should be smooth and easy.

As young people move into adulthood, all services that support them must work closely together so that each person has appropriate support.

Young people with learning disabilities can learn, benefit from learning and should progress to paid employment. The Connexions service provides support and advice to young people about what they want to do in the future. They will organise person centred reviews aged 14-19 that actively involve the person and their family. Part of these plans will also include each young person’s health needs, and one outcome of the review will be a health action plan.

In North Tyneside there are many different pieces of work that are happening with and for young people in transition – Transition DVD, Big Plan, Transition Sub Group, Young People’s Network, Young People’s Advocacy Project, Quality Checkers for young people.
